What is Exposure & Response Prevention Therapy (ERP)?

ERP is an evidence-based treatment approach for Obsessive-Compulsive Disorder (OCD), as well as various anxiety disorders, including social anxiety disorder, panic disorder and specific phobias. ERP involves gradually facing feared situations or thoughts while choosing not to engage in compulsive behaviors. Through repeated practice, individuals learn that anxiety can rise and fall without needing to perform rituals, and that the outcomes they fear are unlikely to happen. This process helps retrain the brain to recognize these experiences as false alarms rather than real danger.

Where is the best place to practice ERP therapy?

Often the best place to practice is in the places where you encounter the fear, such as at home, in your car, or at work. We can achieve this through virtual sessions that support you as you face your anxieties. Real world exposure can be very powerful.

This is not to say that in-office sessions don't have value. They often do, and as you gain practice you'll want to work on addressing your triggers between sessions before fine-tuning the process in the office.

How effective is ERP?

Research indicates that about 7 out of 10 people who participate in ERP therapy with a trained clinician experience significant improvement, often reducing their OCD symptoms by 65–80%. About 3 out of 10 people who complete ERP therapy experience remission, meaning their OCD symptoms no longer significantly interfere with their lives.

These are strong odds and a powerful reason to consider ERP if OCD is causing distress. Many people find that treatment helps them sleep better, feel calmer, and spend far less time managing compulsions.

Do you really need a therapist for ERP?
